Health Takaful in Pakistan (2026): Options Compared

A single hospital admission in a decent private ward in Karachi or Lahore can cost more than many families save in a year. Health takaful spreads that risk through a Shariah-compliant mutual pool, and Pakistan now has real retail options, though fewer than for motor or life cover. Here is what exists in 2026, with actual published numbers where operators disclose them.

How health takaful works

You contribute annually to a waqf pool; the pool pays your hospital bills up to an annual limit, either directly at panel hospitals (cashless treatment against a health card) or by reimbursement elsewhere. The operator charges a wakalah fee for administration. As with all takaful, surplus left in the pool after claims belongs to participants, not shareholders. Cover is annual and renewable, and contributions rise with age.

Pak-Qatar Family Sehat: the most transparent pricing

Pak-Qatar Family Takaful publishes the fullest rate card in the market. Family Sehat comes in six classes with annual hospital care limits of PKR 100,000 (Budget), 200,000 (Economy), 350,000 (Silver), 500,000 (Gold), 1,000,000 (Platinum) and 1,200,000 (Platinum Plus), with daily room and board limits from PKR 2,500 to PKR 55,000. Published sample contributions for ages 91 days to 45 years run from PKR 9,000 per year on Budget to PKR 72,200 on Platinum Plus; ages 46 to 55 pay PKR 13,000 to 126,550. An optional critical illness rider adds a lump sum of PKR 100,000 to 500,000. The network is over 300 panel hospitals with cashless credit via the Pak-Qatar Health Card, plus reimbursement in PKR for accidental emergencies abroad.

Note the pre-existing condition schedule, which is unusually clearly disclosed: conditions you already have are covered at 20 percent of limits in year one, rising by 20 points annually to full cover in year five. Eligibility runs from 91 days old for children and 18 to 59 for adults. If you are managing a chronic condition, this phase-in schedule matters more than the headline limit.

Salaam Sehat: the digital option

Salaam Takaful, Pakistan's largest dedicated general takaful operator, sells Sehat Regular (positioned as affordable protection for individuals, spouses and children) and Sehat Plus (higher limits) through a fully digital journey on the Salaam app, with claims tracking in-app. Salaam publishes less pricing detail than Pak-Qatar, so get a quote and the policy wording before comparing. The company says it returns pool surplus to policyholders every year, though it does not publish the amounts.

Jubilee: health takaful from both sides of the group

The Jubilee group approaches health from two directions. Jubilee Family Takaful offers Takaful Zamin-e-Sehat, a health plan within its family takaful window, alongside its Wellness Assure product. Jubilee General's takaful window covers group health and managed care on the corporate side. TPL's window also lists health takaful alongside its motor-led book. For individual retail buyers, ask specifically whether the product is individual health cover or a rider on a life plan; the two behave very differently.

Group health takaful through your employer

If your employer offers group health, check whether a takaful option exists. Pak-Qatar serves over 3,000 corporate clients across group family and group health segments, State Life's Tayyab window launched group health takaful for corporates, and EFU Hemayah runs group hospital cash and critical illness schemes. Group cover is cheaper per person than retail and usually waives individual medical underwriting, so it should be your first layer, topped up with a personal plan for family members it excludes.

What to check before buying

Five things decide whether health cover performs when you need it. The annual limit per person, not per family. The daily room and board cap, which silently downgrades you to a general ward if set too low. The pre-existing condition schedule. The panel hospital list in your actual city, checked against the hospitals you would genuinely use. And the exclusions list, especially maternity (often a separate rider with a waiting period), dental, and outpatient care, which most hospital plans do not cover at all.

Also confirm the surplus and renewal terms. Health pools run hot: claims inflation in private hospitals is severe, and operators reprice annually. A cheap first-year contribution that doubles at renewal is not a bargain. Ask for the operator's renewal repricing history in writing. Frequently asked questions

How much does health takaful cost in Pakistan?

Using Pak-Qatar's published rates as the market's clearest benchmark: an adult under 45 pays PKR 9,000 per year for PKR 100,000 of annual hospital cover, up to PKR 72,200 per year for PKR 1.2 million of cover. Ages 46 to 55 pay roughly 40 to 75 percent more. Other operators quote individually, so use these figures as a sanity check on any quote you receive.

Are pre-existing conditions covered?

Not immediately. The standard approach, published clearly by Pak-Qatar, phases cover in: 20 percent of limits in year one rising to 100 percent in year five. Staying with one operator long-term therefore has real value; switching resets the clock. Declare conditions honestly, because non-disclosure is the most common reason claims get refused.

Does health takaful cover outpatient visits and medicines?

Standard hospital plans cover admissions, daycare surgery and, in some designs, pre- and post-hospitalization expenses for a limited window. Routine clinic visits, diagnostics and pharmacy bills are generally not covered unless you buy a specific outpatient rider, which is rare in the retail market. Budget for outpatient costs from your own pocket.

Is health takaful different from a hospital cash plan?

Yes. A health takaful plan pays actual hospital bills up to a limit. A hospital cash plan (like the Rs 8,000 per day hospitalization rider on Askari's Golden Path) pays a fixed daily amount regardless of the bill. Cash plans are supplements, not substitutes, since a single surgery can exceed a year of daily benefits.

Can I get health takaful for my parents?

Age limits bite here: Pak-Qatar's Family Sehat accepts adults up to 59, and pricing rises steeply in the 46-55 band. Cover for parents in their sixties and beyond is scarce in the retail takaful market, and where available comes with tight limits and exclusions. For elderly parents, a dedicated medical savings fund in an Islamic money market fund is often the more realistic plan.
